Please follow these instructions exactly:

1. Download the IDL Application in Word Format

2. Download the IDL Application in PDF Format
3. Complete the Application with your word processing program.
4. Print it and SIGN it.
5. Scan each of the following three documents into JPG format:
• Your completed Application after you sign it.
• Your current state issued Driver's License
• Your passport style Photograph
6. Make sure you scan each document into a JPG format.
7. Email these three documents as JPG attachments to:
   IDL@insightbb.com
   (This email address is only for submission of email applications.)
